    The Breakaway Program

    The Breakaway Program is an innovative recreational respite program providing centre-based activities and excursions for children and young people aged between 5 and 17 years with mild to moderate disability.

    To attend Breakaway, families need to have undergone an assessment by the Young Families Assessment Officer.  During the time of Assessment, Breakaway will be offered to families and a copy of the personal care details will be forwarded to the staff to ensure your child’s individual needs are met at the program.

    Times available:
    The Breakaway program operates on Saturday’s during each month, and some days during school holidays.  The time and age group programs changes each month and families receive a program through the mail – the program works on a booking system where families contact the Coordinator to book their child/ren in for the program they are most interested in attending.

    Please be aware that not every program will be suitable for every young person.  Allocation of programs will be made at the discretion of the Breakaway Program Officer.

    Cost of service and payment:
    The cost of the service is $6.00 per program and any additional costs if the program attends a venue or activity. 

    Allocation of service:
    During the time of Assessment, Breakaway will be offered to eligible clients.  Once family give permission for their child to attend the program, they will book in to the programs that most interest them according to the programs that are mailed out to families.  Contacting the Breakaway Respite Program Coordinator makes bookings.

    What are personal care details?
    The personal care details are obtained through the process of assessment or review, which is agreed to by the client, the carer (if applicable) and the service provider.  A copy of this is passed onto the Breakaway program staff to ensure your child’s individual needs are met.  It will include details of:

    ·  Emergency contacts
    ·  Specific/personal care needs
    ·  Family background

    Feedback:
    The staff who work at the Breakaway Program have a duty of care to the clients and their welfare.  They are able to give feedback to the Breakaway Program Officer to support you and your family in caring for your children.

    Cancellation:
    If you have booked your child into a Breakaway program, please contact prior to the program of a cancellation, otherwise you will be fully charged for the program cost.

    Signing in and out:
    It is a requirement that your child is signed in and out of the program each day including contact phone numbers.

    Medication:
    If your child requires medication on any one day, please ensure that the medication book is completed on your arrival at the program and at the end of the day to check the correct time the medication was administered.

    Health and safety issues:
    It is important that if your child is unwell they do not attend the Breakaway Respite Program.  This is to ensure the other children and staff does not contract any illness or communicable diseases.

    Discontinuing service:
    If you no longer wish to use the Breakaway Program you can cancel at anytime by telephoning the Young families Assessment Officer on 9742 0874.  Our assessment officer will appreciate discussing the matter with you, as there may be other ways in which we can help.  You are welcome to ring back at anytime if you feel your circumstances change and you require assistance again.  Any future referrals will not be affected by previous outcomes.
