Disability Support Services at CREATE support people with a disability to achieve successful participation in their community.
Their vision is to:
"Disability Support Services will have a flow through effect, with young people accessing our service to receive support in the areas of training in prevocational, vocational, self advocacy, self esteem, support and encouragement, to assist them in the life areas of their choice."
Their mission statement is:
To Provide Support to people to help them achieve their dreams.
When necessary and appropriate, develop programs and funding support to develop services for people who fit within a service gap
To assist people to map their goals and transition pathways.
CREATE Disability Support Services does not operate under a large group based model (day program). We choose to provide individualised planning and skill development which is in line with our vision and the State Disability Plan. This is achieved by giving you a direct one-on-one worker who will develop goals with you in a flexible way. Therefore our service is not tied in to a 9am-3pm group based facility.
If you choose to attend CREATE Disability Support Services we will assist and support you in developing a plan to meet you needs and goals.
